If you think about it, seeing people smoking at the hospital and drinking when they were still 12 or 13 was pretty normal. Health regulations weren't well-established, and people were not wary of the risks these addictions can do to them.
Not to mention lead paint was used a lot. The government didn't get to regulate the widespread use of toxic chemicals around the environment children would grow up in.
